UMID registration and Application for UMID railway

UMID Card

You can visit the official website of UMID or download the UMID mobile app to initiate the UMID registration process.

For registration, you will need to select the type of beneficiaries first; Employee, Pensioner, and others. You will require to provide your identity particulars like PAN, Date of Birth, PF number, etc., to avail for UMID login.

For initiating the UMID application, first, you will require to login using OTP. Then you can begin with Initiate New Application and fill up all the required details of self and family members. Next, you just need to upload all the necessary documents and submit them.

Checklist for UMID Application

Checklists for UMID differ from employees, pensioners, and their dependents.

Below is the list of documents that all the employees are required to get UMID cards:

Registration for UMID as Pensioner and their Dependents

Your first to get a UMID card is to register on the UMID web portal. Further, you will need the following details to complete the registration:

  • PPO Number (7th CPC)
  • Date of Birth
  • Date of Cessation
  • Pensioner Type (Primary/Secondary)
  • Pension Scheme (with/without FMA)

Complete the registration process before proceeding for an online application for UMID.

For Pensioners and their dependents, You will need to fill the following fields to complete the application form to get a UMID card.

For any complaints or queries regarding your UMID medical cards, you can raise a service request to the UMID help desk. You will need to provide your registered email id along with the subject of your application. Further, you require to state your queries or problems in detail. Next, submit your service request and note the reference number for your request for future use.
